Scene 13: The Flight Home

On the plane, Alex looks back. The island is gone—replaced by a fog bank. But in the window reflection, Ellie's face appears for half a second, smiling slightly. Not forgiving. Just seeing.

Dr. Vance's final voiceover: "Regret Island doesn't exist on any map. It exists in the space between the choice you made and the one you wish you had. You will visit again. Everyone does. The only question is: Next time, will you bring a lifeboat, or will you finally learn to swim with the anchor?"

The screen goes black. Then, a single line of text:

Scene 1: The Departure Lounge (Reality)

The game opens not on the island, but in a sterile, humming airport. Rain lashes against floor-to-ceiling windows. Our protagonist, Alex Mercer (customizable, but canonically a late-30s architect), stares at a boarding pass. The destination is faded: "Haven Island." The voiceover begins, flat and clinical: "They say you don't choose to go to Regret Island. You just stop running."

We learn Alex has been invited to a "therapeutic retreat" for people who have experienced a catastrophic loss. The catch? It was organized by Dr. Elara Vance, a controversial psychologist who believes guilt is a physical anchor. The other passengers in the lounge are silhouettes—until they step into the light.

Scene 2: The Manifest

In a slow, dialogue-heavy sequence, we meet the key players:

The banter is sharp, defensive. Marcus calls it "rich-people self-flagellation." Lina scrolls her phone, but her reflection shows a corpse. The flight is announced. The plane is a small, rickety seaplane. The pilot says only: "Weather's fine. It's the island that's unstable."

Scene 3: The Descent (First Supernatural Glitch)

Mid-flight, the plane hits turbulence. Then, the first "glitch": for exactly 1.5 seconds, every passenger sees a version of their own worst memory projected onto the opposite seat. Alex sees a car plunging off a bridge. Marcus sees a wall of fire. Lina hears a distorted gasp. Scene 13: The Flight Home On the plane, Alex looks back

The lights flicker. Dr. Vance, calm as still water, says: "Welcome to the threshold."

The plane lands on a dark lagoon. The island looms: a jagged tooth of black rock and weeping willows, veined with bioluminescent blue moss. The sign at the dock reads: "Haven Island Resort" — but the word "Haven" has been violently scratched out, and beneath it, carved in crude letters: "REGRET."


Most scenes are gated behind two main variables: Trust and Corruption. To see all scenes, you must play two distinct routes.

Deep below the island, bioluminescent fungi spell out alternate timelines. Each glowing patch shows a parallel scene: the kiss you didn’t take, the job offer you refused, the apology you owed. Interactive element: touching a fungus lets you “live” 30 seconds of that reality—fully rendered, with haptic feedback for supported devices. But the cave echoes with your own heartbeat. : The 6th edition is available in print form.
The 6th edition is 150 pages longer then the previous edition. It now has 754 pages of urology content. The book became too thick to be managed effectively as a single volume using a spiral binding. So, the book has been separated into 2 volumes: oncology and non-oncology (each volume is spiral bound). When you purchase the book, you receive volumes 1 & 2 (the volumes are not sold separately).
There are no new chapters, but here have been updates/additions to almost every chapter. For a brief list of the updates, see below.
